Cremation & Burial arrangements

If your family has experienced loss and you are in need of assistance.  Please fill out the Forms below and bring the following documentation with you at the time of your visit.  If you need help with completing the forms please come into the office for assistance.

1. SSN of Deceased (physical copy required) 

2. Death Certificate 

3. Proof of Address 

4. The applicants relation to the deceased as well as their SSN Card required 

5. Proof of any Debt of the deceased. 

6.Supplemental information ie SSI or SSD 

7. Medicaid or life insurance 

8. Funeral expenses statement/bill
